You can view the full list of mandated benefits and services for your plan on the Illinois Department of Insurance website:

• Accident and Health Checklists (Large Group Plans):

• Affordable Care Act (ACA) Checklists (Individual and Small Group Plans):

When you visit these sites:

1. Chose the applicable checklist based on your plan type and year (e.g., HMO, PPO, POS, ACA-compliant plan, ect.)

 

2. Navigate to the "Benefits" sections within the checklist to see the mandated services

 

3. Changes introduced within the selected calendar year will appear in red

   • Check your Member ID card (your plan type is usually listed there).
   • Review your plan documents in your member portal or from your employer.

    

2. Visit the Illinois Department of Insurance website using the links in Where to Find This Information section.

 

3. Select the checklist that matches your plan type and year (Large Group or Individual/Small Group).

 

4. Go to the "Benefits" sections to view mandated services.
